We have updated our application to make it even easier to complete. Download the application, print it, and fill it in. Or if you PDF viewer allows, download it, open it, fill it in, and then print it.

Alternatively, you can complete the application using the online form here SPSF Online Form, or using your phone and scanning the QR code below. Keep in mind when using the online form, if it is not completed in a single session you will have to start over from the beginning when you return to the form with the information required.

However you decide to complete the application, print it and bring it by UA Cossatot and ask for Erika Buenrrostro. You can also drop it off at the Sevier County Housing Authority at 304 S. 13th Street in De Queen. Ask for Jamie or Shauna and they will be happy to accept your application.

Note: The online form mentions that you can send it using your email, however, keep in mind that email is not secure and you are submitting personal information which if captured by hackers could be used to steal your identity. By submitting via email, you indicate that you are aware of the risks and assume all responsibility for them.
